Before administration

The clinic should know that the right patient is receiving the right clinic-approved treatment under the right authority.

  • Verify identity, history, allergies, medications, and contraindication review.
  • Confirm the required order, authorization, supervision, and consent.
  • Review ingredients, concentration, compatibility, preparation, and expiration.
  • Document baseline observations required by the clinic's protocol.

During and after treatment

Document the administration in enough detail to support continuity and review.

  • Start and completion times, route, site, device, and responsible staff.
  • Ingredients, amounts, lot or batch, expiration, and source when required.
  • Observations, patient response, interruptions, and any intervention.
  • Discharge condition, instructions, warning signs, and follow-up plan.

Inventory controls

Inventory should reconcile to purchasing, preparation, administration, waste, transfer, and expiration.

  • Receive stock with quantity, lot, expiration, and storage requirements.
  • Separate available, quarantined, expired, and recalled inventory.
  • Document use and waste at the point of care.
  • Perform cycle counts and investigate variance rather than silently adjusting it.
